List of documents

All foreigners applying for admission to the university are expected to submit the following documents:


1) a completed personal information/application form;


2) notarized copy of passport with translation into Ukrainian;


3) the original document of education with the transcript of grades for subjects learned with notarized translation into Ukrainian;


4) a medical certificate as to the state of health certified by an official health protection body of the country from where the foreigner has arrived (the certificate to be received not earlier than two months prior to the submission to the university);


5) a copy of the birth certificate;


6) health insurance policy;


7) 12 photos (3х4 mm in size).


The documents indicated in 3), 4), 5) have to be legalized according to existing regulations.
